Our Outreach Community Rehab offers a unique, innovative model of care that sets them apart from any other primary care provider. They reduce or eliminate barriers to care and health disparities, improve health, and lower health system costs. Doing this while empowering communities to actively lead in the direction of their own care. In fact, the success of Outreach Community Rehab is rooted in how they are cultivated and directed. Our main goal is to provide a home visiting therapy service, free of charge, to help local communities. They are required to accept all patients and tailor their services to fit the special needs and priorities of their diverse communities. With active community involvement in care delivery, Outreach Community Rehab offers more than medical care. We offer services that remove persistent common, barriers to health care, such as transportation, translation, insurance enrollment, case management, health education, and home visitation.  Such a broad scope of care is only possible through their utilization of multiple health professionals with varied skills, such as clinicians of all types, community health workers, and team members that are rarely seen in other care settings. The use of multi-disciplinary health care teams can improve access to care, improve patient outcomes, and reduce health disparities. Along with promoting a more efficient and effective primary home therapy care system. The diverse Medical Health Care teams help Outreach Community Rehab to bring high-quality care to a wide variety of patients in the community. There’s a lot of Filipinos facing impairments due to injury or disabilities, which multiply because they receive insufficient education, rehabilitation, and support. As the cycle of poverty continues, people with disabilities often cannot afford to go to any hospitals or rehabilitation centers for treatment. Outreach Community Rehab is a program, designed to provide home-based treatment and holistic care to patients.
Our interns and volunteers will conduct a piece of educational information in local Barangay’s with one thing in mind — to improve the overall health and wellness of local communities. This community-based program will identify common problems that will mobilize resources, develop and implement strategies to reach the objectives we want to accomplish. Our program will encourage an understanding of medical practices, and promote an exchange of medical knowledge. Some of our volunteers possess training within the medical field, and this program will help them gain knowledge and invaluable insight into the healthcare system and procedures within developing countries.
